Continuous Glucose Monitor (CGM) – Dexcom G7 and FreeStyle Libre Medicare Provider Indiana

Life DME expanded its product line to include continuous glucose monitors in 2019.

As a local DME, Life DME’s ability to work closely with the physicians of Northwest Indiana offices to streamline the process in delivery of the Freestyle Libre or Dexcom CGM products to Medicare patients is a relief in comparison to pharmacy benefits which can be very costly as CGM is a DME benefit. Our team will navigate with the patient to ensure that the receiver and sensors are delivered to the patient’s home in a timely manner. Moreover, our office will be in contact with you on a monthly basis for re-order of sensors 5-7 days before your sensor expires in order to ensure that your ability to monitor your glucose levels is constant.

QUALIFICATIONS ARE
1-Diagnosis of Diabetis Mellitus
2-Beneficiary’s treating practitioner has concluded that the beneficiary (or beneficiary’s caregiver)
has sufficient training using the CGM prescribed as evidenced by providing a prescription

CGM is prescribed in accordance with its FDA indications for use

Beneficiary for whom a CGM is being prescribed, to improve glycemic control, meets at least one of the criteria below:

  1. The beneficiary is insulin-treated; or,
  2. The beneficiary has a history of problematic hypoglycemia with documentation of at least one of the following:
    • Recurrent (more than one) level 2 hypoglycemic events (glucose <54mg/dl (3.0mmol/L)) that persist despite multiple (more than one) attempts to adjust medication(s) and/or modify the diabetes treatment plan; or,
    • A history of one level 3 hypoglycemic event (glucose <54mg/dl (3.0mmol/L)) characterized by altered mental and/or physical state requiring third-party assistance for treatment of hypoglycemia

Within six (6) months prior to ordering the CGM, the treating practitioner has an in-person or Medicare-approved telehealth visit with the beneficiary to evaluate their diabetes control and determine that criteria (1-4) above are met.
