Life DME aims to provide respiratory equipment within the Northwest Indiana community by working closely with patients, physicians, sleep labs, hospitals, discharge planners, case management workers, and caretakers to facilitate them and help make their journey easier.

For other insurance coverage, please do not hesitate to send us the referral and we will verify the patient’s benefits to see if out-of-network coverage is available. Medicare , FSSA, Anthem, MultiPlan, Cofinity, PHCS, UnitedHealthcare, UChicago Medicine, UMR Beech Street .
1. We are not contracted with Aetna or Humana, but we can service them if they are a Medicare PPO advantage plan.
2. If we are unable to serve the customer, we will kindly refer them to a supplier who is within their network.
